To schedule an appointment, simply visit DocMJ.com and click ‘BOOK NOW!’ at the top of the page, or schedule your appointment over the phone with one of our Patient Care Coordinators by calling (888) 908.0143. Our office staff can be reached Monday-Friday from 9:00 AM to 8:00 PM and Saturdays from 9:00 AM to 5:00 PM by phone or by email at [emailprotected]. We also offer a Live Chat option on our website. Please note, our administrative office is closed on Sundays. We’re also available on Facebook, Twitter, and Instagram to answer any questions you may have prior to scheduling your appointment. After your appointment date and time is secured, our office will email you a set of electronic intake forms. These forms must be completed at least 24 hours prior to arriving for your appointment, otherwise your exam may be cancelled and a fee assessed to your account. Please review our full Billing Policy for more information. If you need to cancel or reschedule your appointment for any reason, you may do so on our website through the DocMJ Patient Portal. Please review our cancellation policy before cancelling or rescheduling your appointment. You will need to provide our office with medical records or documentation from a licensed medical professional. If you do not have your records on hand or are unable to access them, let us know and our office will be happy to request them for you. You will also need to provide us with your Florida drivers license or State ID card. Please be sure to bring your ID with you to your appointment.

During your appointment, your physician will review your records, intake forms, and will be happy to answer any questions or concerns you may have regarding your condition and medical cannabis. If you are approved for a recommendation, your physician will enter your information into the Florida Medical Marijuana Use Registry, allowing you to apply for your Registry ID Card through the Florida Department of Health’s OMMU (Office of Medical Marijuana Use). Please be aware, the $75 state-required fee for your Registry ID Card is separate from the exam fee you pay to your physician’s office and is due when you apply for your card. According to the OMMU, the average application approval time is currently 7-21 business days. For any questions or concerns regarding your application, you may contact the OMMU directly at (800) 808.9580 or [emailprotected]. DocMJ is unable to provide patients with any application updates as we do not have access to that information.

In order to be eligible for a recommendation for medical marijuana in the state of Florida, you must be a permanent or temporary resident. If you are a temporary resident, you may use any 2 of the documents listed below to prove temporary residency:

Mortgage statement or deed

Utility work order

Utility bill

Mail from any financial institution

Mail from any government agency

Proof of a Florida residential address combined with a supporting statement from the person with whom you’re residing

Other documentation that reflects proof of a Florida residential address

Dr. Glider is a board certified Internal Medicine Specialist. He received his Florida Medical Marijuana Physicians Certification in 2017 and was one of the first recommending physicians with DocMJ. In addition to medical marijuana recommendations, Dr. Glider continues to practice Internal and Geriatric Medicine in private practice.
